Step-by-step how you can nominate a provider:
Log in to your Member Portal.
Navigate to the “Resources” tab.
Click on “Provider Nomination Form.”
Fill out the form as completely as possible.
Why every detail matters
Our Provider Network team uses the information you provide to reach out to the nominated provider. The more complete the form, the faster and smoother the review process. Please make sure to include:
Provider Website – Helps us confirm the provider’s services, location, and credentials.
Office/Practice Name – Ensures we contact the correct business entity.
Doctor/Provider Name – Identifies the specific provider you’re recommending.
Contact Information (phone numbers, emails, etc.) – Critical for our outreach team to connect directly with the provider.
Incomplete information may cause delays or make it difficult for us to successfully recruit your nominated provider into the network.
What to expect next
The nomination and approval process can take a minimum of 4 weeks.
Our team will contact the provider you’ve nominated.
You’ll be notified when we’ve reached out and if the provider decides to join the Beyond Med network.
By submitting accurate and detailed nominations, you’re helping us expand the network and give members like you more access to quality providers and exclusive savings.
